Insurance Policy
We understand that caring for your children’s smiles is your top priority, as it is ours. We will do our best to help you with this. This is why we will help file insurance claims or provide you with an itemized statement for you to submit to out-of-network insurance or your FSA/HSA (Flex/Health Spending Account) for reimbursement.
We kindly ask that you confirm our office is a participating provider with your insurance company or if your insurance company offers out-of-network benefits. Please review the insurance benefits you received from your carrier (often available online). It is the policy holder's responsibility to know if your insurance is active and understand the benefits. If this is not verified before the appointment, you will be financially responsible for all services rendered.
You will receive an estimate of recommended services at your child’s appointment. The estimate is never a guarantee of payment by the insurance company. We encourage you to contact your insurance company if you have any questions or concerns regarding coverage.
You are financially responsible for all charges for services rendered, regardless of insurance coverage. This means that even if you have insurance coverage, you will be responsible for any remaining charges (deductibles and co-pays) and/or services considered non-covered by your insurer.
Financial Policy
Co-pays, deductibles, and/or any non-covered services are due at the time of your visit. You will receive a statement if any balance is due after any insurance payment has been received.
For your convenience, we offer the following methods of payment:
- Cash, Check, Debit Card or Credit Card (Visa, MasterCard, Discover, & American Express)
*There is a 3% service charge on any credit card. This does not apply to debit cards.

Credit Card on File Policy
Craft Smiles Pediatric Dentistry requires parents to keep a credit card on file with our office. We use a secure credit card processing company, nadapayments. When your credit card is entered, your payment information will be stored on nadapayments secure server and available for future transactions. Our office staff will not have access to your card. A valid credit card, debit card, HSA, or FSA card will be accepted.
Your credit card on file will be used to pay account balances after insurance adjudication or provide refunds. Once your insurance processed your claims, they will send an Explanation of Benefits (EOB) to both you and our office showing your total patient responsibility. You typically receive the EOB before we do, so if you disagree with the patient responsibility amount owed, it is your responsibility to contact your insurance carrier immediately.
Our financial policy states you will be responsible for any remaining charges (deductibles and co-pays) and/or services considered non-covered by your insurer. Having a credit card on file is another way to collect that payment. Nothing is changing about how much you pay. You will be sent a statement by mail, email, or text and/or notified by phone before any charges. Any outstanding balances over 30 days will be charged automatically or sent to our profit recovery company.
While completing our registration forms, you will be asked to verify the following:
- You have read the Insurance and Financial Policy above and understand and agree to abide by it.
- You hereby authorize and direct payment of the dental benefits otherwise payable to yourself, directly to Craft Smiles Pediatric Dentistry.
- You agree to be responsible for all charges for dental services and materials not covered by your dental benefits plan unless prohibited by law through a contractual agreement with your insurance company.
- You agree to keep valid credit card information on file securely through nadapayments, and understand that any balances over 30 days will be charged automatically or sent to our profit recovery company.
- You have read and agree to the Appointment Policy, which requires at least 48 hours' notice for any appointment that needs to be rescheduled.
- We REQUIRE CONFIRMATION BY 2PM on the day before an appointment or the appointment will be CANCELED. This will be considered a missed appointment.
- Our office confirms all appointments by email, text, or phone call. We must receive a reply in any of these formats. A $45 charge will be incurred for a missed appointment or if an appointment is canceled with less than 24 hours’ notice.
